• I have previously been diagnosed with irritable bowel syndrome (IBS) by a healthcare professional, and these symptoms are typical for me
• My symptoms are consistent with IBS-related abdominal cramps and have not recently changed, worsened, or become more frequent

• I do not have any diagnosed conditions such as inflammatory bowel disease, bowel obstruction, paralytic ileus, or bowel cancer
• I do not have a personal or family history of bowel cancer or bowel polyps that has not been medically assessed
• I do not have any known allergies to alverine or similar IBS treatments
• I am not taking any medication, and have not been advised by a healthcare professional of any condition, that would make this treatment unsuitable for me

I understand that:
• This treatment is intended to relieve IBS-related abdominal cramps only
• Lifestyle measures such as diet changes, reducing caffeine or alcohol, and managing stress may help improve symptoms
